Once the form has been submitted, we will contact the client to answer any questions they have and begin our 2-week onboarding process if they are happy to sign up. The 3 Step onboarding process is as follows:
Step 1: The Care Admin Team will lodge the clients Home Care Package referral code and send out the self-management onboarding questionnaire to the client or their nominated representative to complete.
Step 2: Once completed, the client will then be sent out a link to book in the care plan meeting. This meeting will be conducted over the phone or video call.
Step 3: Following the care plan meeting, the client’s Care Partner will distribute the care plan and budget to the client/authorised representative and they will be able to provide this to your business to begin organising the care they require. If the box is ticked, Trilogy Care will then notify you once the client’s onboarding process is finalised, and services are ready to start.
